Venice Family Clinic Collaboration
In 2008, Achievable developed a collaboration with Venice Family Clinic, the largest free clinic in the county. Our clients with developmental disabilities can go to Venice Family Clinic on one specially designated afternoon each week to get quality, comprehensive health care from providers who have undergone training in treating individuals with developmental disabilities. The practioners take time to make sure that they are thorough and that patients get all their questions answered and understand their own health.
The care through this collaboration is provided using a medical home model in which patients gain access to continuous, coordinated care. Our patient care coordinator serves as the voice for patients, helping them schedule visits, communicate with the physicians, relay information to family and social workers, understand follow-up instructions and stay on track with their health.
Through our collaboration with Venice Family Clinic, Achievable has been able to provide comprehensive, quality healthcare in an efficient, sensitive manner to over 115 patients thus far. This collaboration paves the way to increased access to quality care for individuals with developmental disabilities.
